Use or adapt this form to track the following:
• Influenza-like illness (ILI): Fever > or equal to 100 degrees F orally AND new cough or sore throat.
• Influenza outbreak: Suspected when three or more cases of ILI are detected during a period of 48 hours.
• Confirmed outbreak: At least one resident has a positive culture or rapid-antigen test for influenza.
