F204483.12(a)(7) (transfer and discharge) requires facilities to:
- Involve the resident and his family, to the extent possible, in selecting the new residence;
- Help orient the resident to the new location. Examples include trial visits by the resident before transfer;
-Work with families to assure the resident he won't leave behind or lose valued possessions;
- Orient staff in the receiving facility to identify the resident's daily patterns;
- Review with staff the routines for handling transfers and discharges in a way that minimizes unnecessary and avoidable anxiety or depression. Staff should recognize characteristic resident reactions identified by the resident assessment and care plan.
- Inform the resident where he is going and takes steps "under its control" to assure safe transportation. Tip: Make sure the resident isn't too sick for the type of transportation selected.
