Preterm labor is the presence of uterine contractions of sufficient frequency and intensity to effect progressive effacement and dilation of the cervix, prior to term gestation (between 20-37 weeks). Preterm labor precedes almost half of preterm births, and preterm birth is the leading cause of neonatal mortality in the United States.

ICD-10-CM Change: To correctly choose your preterm labor code, you need to know:

Did the patient deliver? If not, look at O60.0-.

If the patient has preterm labor and delivers prematurely, you need to know what trimester the preterm labor occurs in and what trimester she delivers in.

If the patient has a term delivery with preterm labor, you need to know what trimester the preterm labor began. Remember, a term delivery, by definition, is greater than 37 weeks 0 days, and so this delivery would always be the third trimester. However, when did the preterm labor start? This would be where you get the third trimester, second trimester, or not specified.

Finally, you must know which fetus this affects to assign your seventh digit.

Documentation: For preterm labor with delivery NOS, report O60.10X-. Remember to assign a 7th digit of ‘0’ for a singleton pregnancy, but a ‘1-9’ if this is a multiple gestation.  Note that an ‘X’ is also required as a place holder since the base code is only 5 characters.
