The Hospice Abstraction Reporting Tool v1.4.0 is now available for download at www.cms.gov/Medicare/Quality-Initiatives-Patient-Assessment-Instruments/Hospice-Quality-Reporting/HIS-Technical-Information.html. The new version includes updated data specs and new notifications, among other changes.
HART is a free software option for "hospice facilities to collect and maintain facility, patient and HIS Record information for subsequent submission to the appropriate national data repository," CMS notes.
