In addition to the hospital readmission measure, the Centers for Medicare & Medicaid Services has proposed these measures for IMPACT implementation, it said in the Feb. 25 Special Open Door Forum:
• Percent of patients/residents with an admission and discharge functional assessment and a care plan that addresses function
• Percent of Residents or Patients with Pressure Ulcers That Are New or Worsened (NQF #0678)
• Percent of Residents Experiencing One or More Falls with Major Injury (NQF #0674).
