A claims glitch that home health agencies thought was fixed appears to be back. HHH Medicare Administrative Contractor CGS has identified Requests for Anticipated Payment and claims Returning To Provider with reason code 1A005 (patient middle initial is invalid), it says on its claim processing issue webpage. But the claim screen is blank when some claims are viewed. CGS reported the same problem earlier in the fall and said it was fixed in an Oct. 29 system update (see Eli’s HCW, Vol. XXVII, No. 38). CGS now says its researching the issue.
