Private Equity-Backed Company Makes Indiana Buy

Grant Avenue Capital portfolio company Fortis Health has acquired PhysioCare Hospice, a Medicare-certified home health and hospice agency in Indiana, the private equity firm says in a release posted to social media. Fortis operates in that state and Utah.
