Note This NOTR Wrinkle For Concurrent Care Demo Patients

If you’re participating in the Medicare Care Choices Model for concurrent care and have submitted an erroneous discharge date on a Notice of Termination/Revocation (NOTR), you’re going to have to use a manual workaround to fix it.

So says HHH Medicare Administrative Contractor CGS on its website. “Instead, of submitting the TOB 8XB with the Condition Code D0 and Occurrence Code 56 and date, hospices need to contact the Home Health & Hospice Provider Contact Center … for assistance,” CGS instructs in a message to providers. “This type of correction for MCCM participating hospices must be made by the Common Working File (CWF),” the MAC relates.

“When you call, the PCC representative will submit a request to have the discharge date corrected,” CGS explains. There are currently 82 hospices participating in MCCM around the country, the Centers for Medicare & Medicaid Services says on its website.
