If you've been using regional home

If you've been using regional home health intermediary Cahaba GBA's sample form for physician attestations for missing or illegible signatures, it no longer has the contractor's official seal of approval. CMS "has indicated that we are unable to provide this sample form," which Cahaba titled "Sample Attestation Statement for Missing or Illegible Signatures." Therefore, Cahaba has removed the form and all references to it from its website, it says in a message to providers.
