Two new guidance documents may help you document and defend your hospice claims.

HHH Medicare Administrative Contractor CGS has posted two tools, “Hospice Terminal Prognosis: Dementia Due to Alzheimer’s Disease” (one page) and “Hospice Terminal Prognosis: Non-Disease Specific” (two pages). The tools offer clinical guidelines for clinicians to document.

For example: Record a "decline in Karnofsky Performance Status (KPS) or Palliative Performance Score (PPS) from less than 70 percent due to progression of disease," the Non-Disease specific tool encourages.
