Use This MAC's Tips To Fill Out Revalidation 855

Confused about how to fill out the 855 form for your revalidation? A new set of frequently asked questions from HHH Medicare Administra-tive Contractor CGS may help. CGS answered a laundry list of revalidation and 855 questions in its Jan. 26 Ask the Contractor Teleconference (ACT). For example: "HHAs do not have to complete the questions in Section 12 regarding the visit projections or banking information when revalidating their enrollment information," CGS stresses in the FAQs at www.cgsmedicare.com/hhh/education/faqs/ACT_FAQs.html.
