If you’re wondering how you may fit into the changing Medicare landscape that includes Accountable Care Organizations, a new resource may give you some ideas. The Centers for Medicare & Medicaid Services has issued a four-page case study of ACO UnityPoint Accountable Care’s home visit program. The ACO’s program focused on patients discharged from the hospital who weren’t eligible for Medicare’s traditional home care benefit. The ACO, which is comprised of nearly 8,000 providers across Iowa, Illinois, and Wisconsin, used both home health staff and ambulatory care stuff in its program, according to the case study.
