You have until July 1 to use up all your old HHCCN forms, but changing over to the new form is even better, Medicare suggests. Reminder: The Home Health Change of Care Notice “is used to notify Original Medicare beneficiaries receiving home health care benefits of plan of care changes. HHAs are required to provide written notification to beneficiaries before reducing or terminating an item and/or service,” the Centers for Medicare & Medicaid Services says on its HHCCN website. The Office of Management and Budget has approved a new HHCCN Form, CMS-10280, CMS says in a message to providers. The form is unchanged except for a new expiration date of “4/30/2022” on the bottom. “HHAs may continue to use the old form up until July 1, 2019 but we encourage HHAs to begin transitioning to the renewed form,” CMS says.
