Alive Hospice in Nashville is reporting another HIPAA breach, about a year after one involving a successful phishing scam (see Eli’s HCW, Vol. XXVII, No. 27). This time around, Alive Hospice “became aware of unusual activity related to an employee’s email account” and quickly investigated, it says in a release. It found “an unauthorized individual” gained access to the employee’s email account from May 4 to May 6. “While, to date, the investigation has found no evidence of actual or attempted misuse of data, we did determine that the email account affected by this incident contained certain personal information” including Social Security numbers, driver’s license numbers, credit/debit card numbers, Medicare/Medicaid numbers, email and password information, and more. Alive Hospice began sending notification letters to patients July 3 and is offering them support services such as a hotline and credit monitoring.
