The second performance year of the Independence at Home Demonstration, which provides chronically ill patients with a range of primary care services in the home setting, saved Medicare $7.8 million in aggregate, the Centers for Medicare & Medicaid Services says in a release. That’s down from $10.6 million the first year.
Seven of the 15 participating physician groups will receive $5.1 million in incentive pay from the program, which focuses on benes with multiple chronic conditions and functional limitations. That’s down from $5.7 million the first year.
