How will new bundling models affect you? To many providers' relief, the Centers for Medicare & Medicaid Services eliminated two bundling models for hip fracture and cardiac payments and scaled back the Comprehensive Care for Joint Replacement (CJR) Model last year. But now CMS has introduced a new bundling model, although it's voluntary this time. Under the Bundled Payments for Care Improvement Advanced model, "participants can earn additional payment if all expenditures for a beneficiary's episode of care are under a spending target that factors in quality," CMS explains in a release about the new initiative. "BPCI Advanced Participants may receive payments for performance on 32 different clinical episodes, such as major joint replacement of the lower extremity (inpatient) and percutaneous coronary intervention (inpatient or outpatient)," CMS says. While the model is aimed at hospitals and physician practices, it also includes "downstream entities," CMS notes on the BPCI Advanced website. The model that includes 90-day episodes covers 32 types of clinical categories including COPD, CHF, various types of joint replacements, and strokes. CMS is taking applications for the model until March 12. The program will run from Oct. 1, 2018 to Dec. 31, 2023. CMS is holding a question-and-answer Open Door Forum about the initiative on Jan. 30.
