Don't Flub Your Address Requirements On 855
Don’t get your address requirements on the Medicare enrollment form mixed up — because there are a lot of them.
The CMS-855A enrollment application for home health agencies and hospices has no less than 11 places to enter an address, ranging from your correspondence address to your record storage location to your chain home office address, notes HHH Medicare Administrative Contractor Palmetto GBA in a new job aid that locates and describes each place to enter address information.
