Industry Note:

Beware This Claims Payment Snafu For Low-Visit Claims

A system glitch is resulting in remittance advice errors for LUPA claims. "For Low Utilization Payment Adjustment (LUPA) Home Health type of bill 339 final claims, with a through date of April 1, 2013, and after, there is a problem with the electronic remittance advice (ERA)," all three HHH Medicare Administrative Contractors say in messages to the providers they service.

Good news: "The payment amount for these claims is correct as is the summary line information on the ERA," the MACs explain.

Bad news: "The line and claims level information on the ERA is incorrect. As a result, the ERAs are force-balanced," the MACs say.

You can identify these ERAs "easily because they will have the Claim Adjustment Group Code of OA and the Claim Adjustment Reason Code A7, indicating that it has been force-balanced," the MACs’ message says. "These remittances cannot be regenerated in the future to correct their appearance."

Say tuned: "CMS is working to correct this problem," the MACs pledge.
