Beware Claims Returned With Reason Code 12201

Providers having trouble with their roster billing again aren't alone. Providers are seeing 34x type of bills returned to provider (RTP'd) incorrectly with reason code 12201, explains HHH MAC Palmetto GBA on its website. "The Fiscal Intermediary Standard System (FISS) is incorrectly removing the "From" date on claims that are entered via the Roster Bill Entry screen," HHH MAC CGS says on its website. "As a result, the reason code 12201 fails because the statement covers period is invalid." A fix is currently being scheduled, Palmetto assures providers. In the meantime, providers can work around the problem by accessing 12201 claims in RTP, adding the "From" date, and pressing F9, CGS instructs.
