The guilty pleas keep coming in the fraud case involving four Detroit-area home health agencies -- Physicians Choice Home Health Care, First Care Home Health Care, Quantum Home Care Inc. and Moonlite Home Care Inc. Co-owner Muhammad "Sib" Ahmad pled guilty to a $13 million home care scam, the Department of Justice says in a release. The laundry list of misdeeds Ahmad admitted to includes paying non-licensed professionals to pretend to be physicians to Medicare beneficiaries; paying clinical staff to create fictitious files for services never rendered; paying kickbacks to recruiters; having beneficiaries pre-sign forms and visit sheets that were later falsified; and forging beneficiary signatures on such forms. Ahmad also admitted to forming a shell company, Century Home Care, to launder the proceeds. Co-conspirators who also have pled guilty in the scheme include Zahir Yousafzai, Dr. Dwight Smith, and Tausif Rahman.
