Your longer-stay patients’ records will need to prove eligibility in a number of ways, but one hard and fast requirement will be objective measurements. Consider using these eligibility assessment tools offered by physician David Fedor, Medical Director for Palliative Care at McKay Dee Hospital, System Medical Director of Palliative Care for Intermountain Healthcare, and Medical Director for Envision Hospice, in a presentation at the Utah Hospice & Palliative Care Organization’s November convention:

  • Nutritional status measurement tools
  • Weight scales
  • Body Mass Index (BMI)
  • Mid Arm Circumference (MAC)

  • Mini-Mental Exam
