Update Gastrostomy Tube Replacement Coding

Question: Our surgeon performed a percutaneous replacement of a gastrostomy tube. Although the procedure did not require endoscopic guidance, the surgeon did need to dilate the stenosed tract to complete the procedure. We’ve always reported percutaneous gastrostomy tube removal and replacement using 43760, but would we use a different code due to the complications?

Answer: The code you mentioned, 43760 (Change of gastrostomy tube, percutaneous, without imaging or endoscopic guidance) has been deleted.

Fortunately, CPT® replaced the code with two, more specific codes. The code for an uncomplicated percutaneous gastrostomy tube replacement is 43762 (Replacement of gastrostomy tube, percutaneous, includes removal, when performed, without imaging or endoscopic guidance; not requiring revision of gastrostomy tract). But that doesn’t describe your scenario, because you have documentation that the surgeon did revise the gastrostomy tract.

Do this: Report the procedure using 43763 (… requiring revision of gastrostomy tract).
