Question: Encounter notes indicate that the ED physician injected collagenase for a patient with “palmar fascial cord.” After the injection, the physician also manipulated the cord. Is there a way to code for both services?

Answer: Yes, there is. On the claim, report:
- 20527 (Injection, enzyme (eg, collagenase), palmar fascial cord (ie, Dupuytren’s contracture)) for the injection
- 26341 (Manipulation, palmar fascial cord (ie, Dupuytren’s cord), post enzyme injection (eg, collagenase), single cord) for the manipulation
- Modifier 51 (Multiple procedures) appended to 26341 to indicate you are reporting multiple procedures
- M72.0 (Palmar fascial fibromatosis [Dupuytren]) appended to 20527 and 26341 to represent the patient’s palmar fascial cord
More info: “Palmar fascial cord” is also known as Dupuytren’s contracture.
