Separate EKG and X-Ray Documentation

Question: To bill for electrocardiograms (EKGs) and x-rays, do the EKG and x-ray results have to be an identifiable part of the documentation, separate from the E/M-level documentation? In what form should this be included?

Answer: Billing EKG and x-ray interpretations correctly requires documentation that has the same level of detail a cardiologist or radiologist might have provided. The documentation format is incidental. If its documented, its done. But check with your carrier before billing because many require that it be a separate document.
