Question: Where do I find good examples of documentation cases for each level of service in ED? 99291-99285?

Answer: There are clinical examples for each ED E/M code and critical care code in Appendix C, at the back of the CPT® book. The AMA also publishes a book, CPT® Case Studies: Examples of Procedures and Services, which includes a brief case study and descriptions of pre, intra, and post service work associated with the ED E/M codes and common ED procedures. These may be useful to you as examples of patient presentations for each level of ED code available.
