Initial Arrhythmia Attempts Unsuccessful

Question: I have an electrophysiology (EP) study that indicates pacing from the right atrium and ventricle and recording from the atrium, bundle of His, and ventricle. And, the physician mentioned that his initial attempts to induce an arrhythmia failed, but following infusion of Isuprel he was able to induce an arrhythmia. Which codes should I report?

Answer: You should report comprehensive EP code 93620 (Comprehensive electrophysiologic evaluation including insertion and repositioning of multiple electrode catheters with induction or attempted induction of arrhythmia; with right atrial pacing and recording, right ventricular pacing and recording, His bundle recording) for the full study. To report the arrhythmia stimulation after the Isuprel infusion, use +93623 (Programmed stimulation and pacing after intravenous drug infusion [list separately in addition to code for primary procedure]).
