Question: How should I code a combined right and left heart cath that included measuring a patent foramen ovale using intracardiac echo?

Answer: The most appropriate procedure codes for your case are:
- 93453, Combined right and left heart catheterization including intraprocedural injection(s) for left ventriculography, imaging supervision and interpretation, when performed
- +93662, Intracardiac echocardiography during therapeutic/diagnostic intervention, including imaging supervision and interpretation (List separately in addition to code for primary procedure).
Keep in mind: If you have an AMA CPT® 2011 manual, the note under +93662 has a typo, instructing that you may report the add-on code in addition to 93543. It should instead refer to 93453.
