92982 With 92980? Code Stent and Angioplasty

Question: For 2012, can you bill for a stent and angioplasty in the same visit if they were done in two different vessels, such as 93458-26, 92980-RC, 92982-LD? How would you report the same service in 2013?

Answer: You may report the intervention in each main coronary vessel separately, but for 2012 you should report a primary vessel code and then an additional vessel code, rather than reporting the two primary codes mentioned in your question.

For example: For 2012 stenting performed in the right coronary artery and angioplasty in the left anterior descending coronary artery, report these deleted-for-2013 codes:

92980-RC, Transcatheter placement of an intracoronary stent(s), percutaneous, with or without other therapeutic intervention, any method; single vessel; Right coronary artery

+92984-LD, Percutaneous transluminal coronary balloon angioplasty; each additional vessel (List separately in addition to code for primary procedure); Left anterior descending coronary artery.

You should not report 92980-RC and 92982-LD (Percutaneous transluminal coronary balloon angioplasty; single vessel) together. If you do report them on the same claim for your 2012 service, Correct Coding Initiative (CCI) edits will reject 92982.

You may report the diagnostic cardiac catheterization you mentioned separately: 93458-26 (Catheter placement in coronary artery[s] for coronary angiography, including intraprocedural injection[s] for coronary angiography, imaging supervision and interpretation; with left heart catheterization including intraprocedural injection[s] for left ventriculography, when performed).

To indicate the cardiac cath was diagnostic and not part of the interventional services, append modifier 59 (Distinct procedural service) to 93458-26.

2013: Code 93458 will still be valid in 2013, and you may report it at the same session as coronary interventions, assuming the cath meets the rules for a true diagnostic service. In 2013, your coronary stent and angioplasty options will change:

92928-RC, Percutaneous transcatheter placement of intracoronary stent(s), with coronary angioplasty when performed; single major coronary artery or branch

92920-LD, Percutaneous transluminal coronary angioplasty; single major coronary artery or branch.
