Pinpoint Correct Codes for ICD Removal

Question: My provider removed an existing implantable cardioverter-defibrillator (ICD) from the pocket. They removed one malfunctioning lead and replaced it with a new lead. They then placed the old/current ICD generator back in the pocket. How should I report this claim?

Answer: You should report 33216 (Insertion of a single transvenous electrode, permanent pacemaker or implantable defibrillator) for the insertion. You should also report 33244 (Removal of single or dual chamber implantable defibrillator electrode(s); by transvenous extraction) for the removal of the lead.
