Reader Question:

Use New Code for Bilateral Iliac Wall Graft Stenting

Question: Do 37205 (Transcatheter placement of an intravascular stent[s], [non-coronary vessel], percutaneous; initial vessel) and +37206 (... each additional vessel [list separately in addition to code for primary procedure) cover the stents for bilateral iliac aneurysms, or should I use an unlisted-procedure code? Nevada Subscriber Answer: CPT 2003 includes a new code for iliac aneurysm repair: 34900 (Endovascular graft replacement for repair of iliac artery [e.g., aneurysm, pseudoaneurysm, arteriovenous malformation, trauma]). Report 34900 and append modifier -50 (Bilateral procedure) because the repair procedure was bilateral.

Use 75954 (Endovascular repair of iliac artery aneurysm, pseudoaneurysm, arteriovenous malformation, or trauma, radiological supervision and interpretation) and append modifier -26 (Professional component) for the radiological supervision and interpretation if the procedure was done in the hospital rather than the office. Also report the appropriate catheter placement codes, depending on the physician's access site and method.
