Reader Question:

Take a Look at This Cardiac Cath Documentation

Question: According to the documentation, after anesthesia was obtained, my cardiologist entered the patient’s right radial artery using the modified Seldinger technique and placed a 5 Fr. introducer side arm sheath in the artery for access. They used the following cardiac catheters: 5 Fr. Jacky, JL 3.5, and 5 Fr PA. My cardiologist performed coronary arteriography using multiple injections of contrast in multiple angulated views. They performed left ventriculography in ROA view. Hemodynamics for both right and left heart were documented as well as findings of the LV and coronary arteries. Does the documentation support selective angiography? I was debating between 93453 (Combined right and left heart catheterization including intraprocedural injection(s) for left ventriculography, imaging supervision and interpretation, when performed) or 93460. Can you help me?

Answer: Yes. Because there are documented findings of the coronary arteries, you can report 93460 (Catheter placement in coronary artery(s) for coronary angiography, including intraprocedural injection(s) for coronary angiography, imaging supervision and interpretation; with right and left heart catheterization including intraprocedural injection(s) for left ventriculography, when performed).
