This classification may be integral to ICD-10-CM code choice starting in October.
ICD-10-CM code choice doesn’t quite line up with the current myocardial infarction (MI) classification, but that may change soon.
Background: A proposal for a change to ICD-10-CM explains that the ACC/AHA Task Force on Data Standards developed a myocardial infarction classification “as a controlled terminology for the purpose of interoperability among electronic health information systems.” The request is to have the codes match the classification.
The listed presenters were David Berglund, MD, and James E. Tcheng, MD, FACC, Professor of Medicine, Duke University School of Medicine, Member, ACC/AHA Task Force on Clinical Data Standards, and Vice Chair, Writing Committee to Develop Cardiovascular Endpoints Data Standards.
Overview: The changes would add inclusion notes to STEMI and NSTEMI codes I21.0 to I21.4 to specify that those codes are appropriate for type 1MIs, which are spontaneous MIs.
The proposal then adds a new subcategory for the other types of MIs. They suggest code I21.A1 for type 2 MI (due to demand ischemia or ischemic imbalance) and I21.A9 for the remaining types 3, 4a, 4b, 4c, and 5.
