NGS Providers: Take a Close Look at PCI Denials

You may need to add a coronary modifier and resubmit your claim.

Offices in Jurisdictions K and 6 need to take a look back through their percutaneous coronary intervention (PCI) claims.

National Government Services (NGS), the Part B MAC for those jurisdictions, advises providers that coronary artery modifiers are required on specific coronary procedure codes.

The coronary modifiers are listed below:

  • RC, Right coronary artery
  • LC, Left circumflex coronary artery
  • LD, Left anterior descending coronary artery
  • LM, Left main coronary artery
  • RI, Ramus intermedius.

NGS providers should append the coronary modifiers to these codes:

  • Coronary revascularization: 92920, 92924, 92928, 92933
  • Revascularization of or through graft: 92937
  • Coronary/graft revascularization during acute myocardial infarction: 92941
  • Coronary/graft revascularization of chronic total occlusion: 92943
  • Coronary mechanical thrombectomy: +92973
  • Coronary thrombolysis: 92975
  • Coronary/graft intravascular ultrasound: +92978, +92979
  • Coronary/graft flow reserve measurement: +93571, +93572.

NGS updated its PCI LCD (L28395) on Feb. 1, 2014, to include these instructions.

Action point: NGS advises providers to resubmit claims that were denied because of the missing modifiers. But this time around, be sure that modifier is where it belongs.
