Reporting J2795 and 64450 Together

Question: When we file J2795 with 64450, we get denials stating the codes are bundled. Is J2795 truly bundled into 64450?

Answer: According to the latest Correct Coding Initiative (CCI) edits, codes J2795 (Injection, ropivacaine hydrochloride, 1 mg) and 64450 (Injection(s), anesthetic agent(s) and/or steroid; other peripheral nerve or branch) are not bundled, so you can submit them on the same claim. However, don’t expect payment for both.

Code J2795 represents ropivacaine hydrochloride, an anesthetic medication that blocks nerve impulses that send pain signals to the brain. According to the Outpatient Fee Schedule, the payment indicator for J2795 is N1, which states: Packaged service/item; no separate payment made. Because of this classification, ropivacaine hydrochloride drugs are packaged into the procedure and bring no separate reimbursement.

You should report both codes to give a clear picture of the injection procedure your pain management specialist administers, but you cannot receive additional payment for medication with J2795.
