If the criterion of "by other than attending physician" doesn't fit your situation, you should report 59899 (Unlisted procedure, maternity care and delivery) instead of using a code from another CPT section. The corresponding anesthesia code is 00940 (Anesthesia for vaginal procedures [including biopsy of labia, vagina, cervix or endometrium]; not otherwise specified).
