Question: What CPT code can I use for an ischial tuberosity bursa injection?

Answer: The correct choice is 20610 (Arthrocentesis, aspiration and/or injection; major joint or bursa [e.g., shoulder, hip, knee joint, subacromial bursa]). If your provider uses imaging guidance, you'll code separately for that part of the service. Options could include 76942 (Ultrasonic guidance for needle placement [e.g., biopsy, aspiration, injection, localization device], imaging supervision and interpretation), 77002 (Fluoroscopic guidance for needle placement [e.g., biopsy, aspiration, injection, localization device]), or others.

Background: A person with ischial bursitis has inflammation of the bursa that lies over the ischial tuberosity (the bony prominence in the pelvis that we sit on). Report ischial bursitis with ICD-9 code 726.5 (Enthesopathy of hip region). Enthesopathies are disorders of peripheral ligamentous or muscular attachments. When ICD-10 becomes effective, you will instead report M70.7- (Other bursitis of hip) with the fifth digit indicating unspecified hip (M70.70), right hip (M70.71) or left hip (M70.72).

Treatments for ischial bursitis can include rest, use of a doughnut cushion while sitting, and pain medications. Severe cases might require corticosteroid injections over the ischial tuberosity.
