Question: Our pain management specialist administered a cervical facet zygopophyseal intraarticular joint injection. He used fluoroscopic guidance. What is the correct CPT® code?

Answer: You should report 64490 (Injection(s), diagnostic or therapeutic agent, paravertebral facet (zygapophyseal) joint (or nerves innervating that joint) with image guidance (fluoroscopy or CT), cervical or thoracic; single level). If the provider had used a different type of guidance such as ultrasound, you would report 0213T (Injection(s), diagnostic or therapeutic agent, paravertebral facet (zygapophyseal) joint (or nerves innervating that joint) with ultrasound guidance, cervical or thoracic; single level) instead.
