Code 36569 Normally Applies to Power Wand Usage

Question: Our anesthesiologist is investigating using the power wand. What exactly is it for, and how would we code for it?

Answer: The power wand is an extended, indwelling peripheral IV catheter that physicians use for infusions, long-term antibiotics, and similar situations. The catheter tip ends in the brachial, basilica, or cephalic vein, distal to the shoulder (axillary vein). It's considered a midline catheter.

Code 36569 (Insertion of peripherally inserted central venous catheter [PICC], without subcutaneous port or pump; age 5 years or older) is the appropriate choice for most patients, if the anesthesiologist performs the procedure. If the power wand placement is a second line item on the claim, append modifier 59 (Distinct procedural service) to 36569 to distinguish it from the procedural anesthesia.

If the anesthesiologist provides anesthesia for another physician to place the catheter, you should report 00532 (Anesthesia for access to central venous circulation) instead of 36569.
