Choose J0475 Over J3490 for Baclofen Injection

Question: Our provider used a combination of baclofen, sodium chloride, and sterile water during a baclofen pump refill. Should we report J0475 or J3490 for the procedure? Virginia Subscriber Answer: The HCPCS code for 10mg of baclofen is J0475 (Injection, baclofen, 10mg). This is your better option since insurance wouldn’t recognize which drug is being injected if you report J3490 (Unclassified drugs). Because J0475 is based on a 10mg dose, report it for each 10mg of baclofen injected.
