Question: How should I report an abdominal aortogram with bilateral runoffs?

Answer: Before you code anything, make sure the procedures have followed appropriate guidelines. To report an abdominal aortogram with runoff, the catheter must be positioned in the upper abdominal aorta and the physician must study the abdominal aorta and extremities through at least the femoral arteries in one fluid exam. For this procedure, report 75630 (Aortography, abdominal plus bilateral iliofemoral lower extremity, catheter, by serialography, radiological supervision and interpretation). Note: A unilateral extremity study does not equate to runoff.
If the study did not include runoff examination, you should report 75625 (Aortography, abdominal, by serialography, radiological supervision and interpretation). The physician may indicate this procedure with the phrase “flush aortography.”
