You should report chemodenervation injections by the site the physician treats:
Anal sphincter: 46505 (APC 0148) -- Chemo-denervation of the internal anal sphincter
Eccrine glands:
• Axillae, 64650 (APC 0204) -- Chemo-denervation of eccrine glands; both axillae
• Hands and/or feet, 64999 -- (APC 0204) Unlisted procedure, nervous system
• Other area(s), 64653 (APC 0204) -- Chemo-denervation of eccrine glands; other area(s) (e.g., scalp, face, neck), per day (This code applies "per day," not per injection or per muscle or muscle group)
Extraocular muscle: 67345 (APC 0238) -- Chemo-denervation of extraocular muscle
Extremity muscle: 64614 (APC 0204) -- Chemo-denervation of muscle(s); extremity(s) and/or trunk muscle(s) (e.g., for dystonia, cerebral palsy, multiple sclerosis)
Facial muscle: 64612 (APC 0204) -- Chemo-denervation of muscle(s); muscle(s) innervated by facial nerve (e.g., for blepharospasm, hemifacial spasm)
Neck muscle: 64613 (APC 0204) -- Chemo-denervation of muscle(s); neck muscle(s) (e.g., for spasmodic torticollis, spasmodic dysphonia)
Trunk muscle: 64614 (APC 0204) -- Chemodenerv-ation of muscle(s); extremity(s) and/or trunk muscle(s) (e.g., for dystonia, cerebral palsy, multiple sclerosis).
