Project Xtern - Xtern Evaluation Form

This form can be used by our AAPC-AOES Project Xtern facilities to evaluate your AAPC Project Xtern Members performance. Please be sure to evaluate your extern at least once at the completion of the externship. If you use this form or create one of your own, be sure to give your extern a copy of your evaluation so that they can utilize this tool for future employment opportunities.
